1. Can your organisation name, in writing, every AI system that is currently making or materially influencing consequential decisions about customers, employees, or suppliers?
2. For each AI-supported decision, has your organisation explicitly documented whether the decision is AI-made, AI-assisted, or AI-informed — and who the human accountable owner is for each category?
3. When an AI system produces a consequential error, how quickly can your organisation identify the accountable person and activate the remediation process?
4. At your last board meeting where a digital transformation investment was approved, did the approval include an explicit governance framework for how the system’s consequential decisions would be accountable?
5. Can your senior leadership answer, without referring to the technology team, what decisions a specific AI system is making — and on what basis?

6. For your most consequential AI system: do you know what population the training data came from, and how representative that population is of the people the system now makes decisions about?

8. Who conducts your AI bias audit, and how independent are they from the team that deployed the system?
9. When a bias has been identified in an AI system, what is your organisation’s documented response process?
10. To what extent is “what is this system optimising for, and whose definition of a good outcome does that encode” explicitly asked as a governance question before deployment?

11. When data is presented in support of a strategic decision in your organisation, how often is the quality and provenance of that data formally challenged before the decision is made?
12. How would you characterise the dominant relationship between data and decisions in your organisation — does the data drive the decision, or does it legitimise a direction already chosen?
13. Does your senior leadership team have sufficient data literacy to distinguish between Level 2 (performance benchmarks) and Level 3 (genuine intelligence) in the data they receive?
14. How is data protection and sovereignty compliance managed for AI systems handling personal data in your organisation’s operating markets?
15. Is there an internal process for evaluating whether AI output is being used at the appropriate Intelligence Pyramid level — or whether strategic decisions are being made on operational data presented as intelligence?

18. How would you rate your senior leadership team’s ability to interrogate AI vendor claims — to ask the right questions about model limitations, training data, and performance conditions?
19. Has your organisation conducted a formal digital leadership development programme — not technology training — specifically addressing AI governance literacy for the senior team and board?
20. If a significant AI governance failure occurred at your organisation tomorrow, how confident are you that your board could provide a credible public account of the governance process that should have prevented it?
